Nursing Home Quality Report for Professionals
Nursing Care Facility
Scoring of Compliance
Each nursing home received seven scores:
1. Recent licensing survey performance
2. Prior licensing survey performance
3. Second prior licensing survey performance
4. Third prior licensing survey performance
5. Complaint survey performance
6. Repeating quality of care deficiency performance
7. Repeating non-quality of care deficiency performance
Licensing survey performance scores were computed by totaling the values determined using the Scope and
Severity grid of all deficiencies noted in each survey. The complaint survey performance score was computed by
totaling the values of the deficiencies noted in any complaint survey.
Repeating deficiency performance scores were computed by analyzing the frequency of the deficiencies and the
impact deficiencies had on quality of care. To analyze deficiency frequency, HealthGrades assigned a numerical
value depending upon the number of times a deficiency was repeated. For example, deficiencies could be
repeated two, three or four times. Repeating deficiencies that were related to quality of care were assigned higher
numerical values than other repeating deficiencies. Each nursing home received a score for repeating deficiencies
that related to quality of care and a score for all other repeating deficiencies.
Statistical Analysis
HealthGrades used principal component analysis in the statistical package, Minitab 12 to obtain a weight for each
of the seven scores. Any nursing home with a score in any of the seven areas that was more than four standard
deviations from the mean was not included in the statistical analysis.
Nursing Home Ratings
Using the seven assigned weights from the statistical analysis, an overall score was computed for each nursing
home. The overall scores ranged from negative to positive values. Nursing homes with large negative scores
were those nursing homes that did not perform well in one or more of the seven areas analyzed. Based on each
nursing home's overall score, HealthGrades applied the following rating system:
Best 30% of all nursing homes within each state
Middle 40% of all nursing homes within each state
Bottom 30% of all nursing homes within each state
