HealthGrades Quality Ratings Overview
HealthGrades quality ratings are derived from data provided by HCFA in the Online Survey Certification and
Reporting (OSCAR) database and SNF Complaint database. Utilizing HealthGrades' proprietary methodology,
each of the following elements received a numerical score:

Most recent licensing survey deficiencies
l
Prior licensing survey deficiencies
l
Second prior licensing survey deficiencies
l
Third prior licensing survey deficiencies
l
Deficiencies found in two or more surveys not related to quality of care
l
Deficiencies found in two or more surveys related to quality of care
l
Complaint survey deficiencies (effective 2001 ratings)
HealthGrades utilized statistical modeling to calculate the overall quality score. The scores ranged from
negative to positive values, with large negative scores reflecting poorer performance and positive
scores reflecting better performance.
The star rating was based upon the overall score.
