Based on the 2003 National Inpatient Sample (previously 1997).

No longer risk-adjusted by AHRQ. HealthGrades applied a risk-
adjustment based on age, gender, and DRG.
Decubitus ulcer
·
Excludes patients transferred from an acute care facility.
·
Excludes patients with a procedure for debridement or pedicle graft
before or on the same day as the major operating room procedure
(surgical cases only).
·
Excludes patients with a diagnosis code of spina bifida or anoxic brain
damage.
Foreign body left during procedure
·
No longer risk-adjusted (was previously adjusted for age, gender,
DRG and comorbidity categories).
Iatrogenic pneumothorax
·
Excludes patients with a diagnosis of diaphragmatic surgery repair
and pleural effusion.
Post-operative physiologic and metabolic
derangements

Excludes patients with chronic renal failure.
