In general CPT codes describe what you did in a clinical encounter and ICD-9 codes
describe why you did it. The ICD-9 codes will be listed in the diagnosis portion of the
claim. One of the counseling codes (99402 or 99403) may be used for the level of care
code if the patient has been referred and a report generated to the referring physician.
V65.8 may be used for diagnosis of preventative care or V01.9 for exposure to infectious
disease. Many third party and Medicare coverage for these codes is rejected.

Vaccine Information Sheets are available on the CDC website. The March 1988
Childhood Vaccine Injury Act requires that immunizations be recorded in the patient's
chart with the following information: administration date, vaccine name, vaccine
manufacturer, lot number, and the administrators name, business address, and title. It is
recommended that the site and route of administration and the expiration date of the
