Client Record Card
/
Release Form
The key to successful client relationships and maintaining ongoing hair color
services and consistent chemical results is to keep accurate records. The
client record card contains information such as the client's name, address,
and telephone number. The client record card also contains information
about the condition of the hair and scalp, especially noting if the client is on
any type of medication or taking vitamins. The client record card is used
from the consultation period up until the client has left the salon.
A client release statement helps the school or
salon owner avoid retribution from any damages
or accidents and may be required by some mal-
practice insurance.
However, it is not a legal docu-
ment and may not absolve the hair colorist from any
damage that may occur to the client's hair as a result
of the chemical service.
