P reparing Entry Forms
Please complete the following two forms (type or neatly
print) and submit with your work:
1. Entry Label: Attach one copy to your transparencies or disk
and attach another copy to your payment form.
CRITICAL: The information you provide on the Entry Label may
be used for publication. Fact check and copy proof all names
and information.
2. Payment Form: Include all business and payment informa-
tion on this form.
CRITICAL: Attach one copy of each Entry Label, which you have
attached to your transparencies or disk, to the Payment Form.
Attaching Entry Labels
Copy entry labels as needed, but do not enlarge, cut or alter
Entry Label to fit on back of work or disk. For transparencies/
slides, tape the entry label to the upper left hand corner of the
slide carrier--behind the slide. For CD-Roms or disks, tape label
securely to the back, fold the bottom half of the label under so
that the label is neatly attached. For printed work, tape the top
and bottom of the label to piece. If the label is larger than the
work, then fold the bottom half of the label under--so that it
neatly fits.
Packing Your Entry
· Label all slides and disks clearly with your name, company
and telephone number.
· Enclose a separate payment form and payment for each book-
do not submit to multiple books on the same payment form.
· Put payment form with attached entry labels in separate enve-
lope marked "payment" and pack with entry.
· Overseas entrants: Write "Art for contest. No commercial
value." on the outside of the package.
Sending Your Entries
Please note the title of the book you are submitting to when
addressing your package, and deliver your entries by the dead-
line date to Graphis's New York office.

Confirmation of Receipt
If you re q u i re proof of delivery, enclose a self-addressed, stam p e d
p o s t c a rd with your entry. International entrants add $1 US for
postage to total entry fee. The postcard will be mailed to you
when your entry is processed. Entries take up to 6 weeks after
the deadline date to process. You may also wish to ship your
entry via an overnight delivery service or re g i s t e red parcel post
with a signature re q u i red delivery. Due to the volume of entries,
Graphis cannot confirm receipt of entries over the telephone.
Notification of Selection
Graphis notifies entrants 6 to 8 months after the submission
deadline, whether or not their work has been selected.
Book Discounts
With their submission, every entrant has the opportunity to pur-
chase books at a one time discount of 20%. Please see pay-
ment form for details. Entrants whose work is selected will
receive the opportunity to buy additional copies of the publica-
tion at a discount with their notification of selection.
