By entering the Crossroads Film Festival, I understand clips, segments, photographs
and/or stills from my entry may be used in the promotion of the Crossroads Film Festival.

1. Completed Entry Form (One Entry Form per submission. Work entered in multiple
categories must be accompanied with its own Entry Form).
2. NTSC VHS preview copy of submitted film.
3. Entry fee.
4. Filmmaker(s) biography/resume.
5. Production credits of the film.
6. Press materials, production stills, and other media/promotional items (optional).
7. Photograph of filmmaker, if available.
If you would like your entry tape returned, please include a self-addressed stamped
envelope. The Festival will not automatically return tapes.
ENTRIES MUST BE POSTMARKED NO LATER THAN DECEMBER 31, 2002
LATE ENTRIES WILL BE ACCEPTED THROUGH JANUARY 31, 2002
