19.05±0.55 (_ <0.01). Conclusion: In our study a method Doppler - ECHO have shown good
correlation with the data of right heart catheterization (r=0.83).

EFFECT OF SILDENAFIL ON PULMONARY HEMODYNAMICS IN PATIENTS WITH

Background: We have demonstrated that sildenafil has a significant inhibitory effect on
hypoxia-induced pulmonary vasoconstriction in healthy subjects (Zhao et al., 2001). Aim: To
investigated the effect of 50 mg dose of the selective phosphodiesterase 5' inhibitor sildenafil on
pulmonary hemodynamics in patients with high altitude pulmonary hypertension (HAPH).
Methods: Fourteen patients with HAPH (13 men and 1 women), 28-72 years old (55.85±2.89),
were studied in National Center of Cardiology, Bishkek (760 meter above sea level). Pulmonary
artery pressure (PAPm) was measured by right heart catheterization. Baseline measurements were
made after 30 minutes rest, and than 50 mg of sildenafil were given orally. Measurements of
PAP, systemic blood pressure, heart rate, SaO2 were repeated in 1 hour later. Results: In 1 hour
after sildenafil the mean PAP significantly decreased from 33.92±1.96 mm Hg to 27.28±1.67 mm
Hg (p<0.01) and systolic pulmonary artery pressure decreased from 49.5±3.62 mm Hg to
40.35±2.98 mm Hg. Whereas systemic blood pressure, heart rate, wedge pressure, SaO2 were not
significantly changed. Conclusion: 50 mg acute dose of sildenafil significantly decreases mean
PAP, does not have the systemic effect and could be recommended for treatment of HAPH.

Background: We have demonstrated that sildenafil has a significant inhibitory effect on

investigated the effect of the chronic treatment by sildenafil in patients with high altitude
pulmonary hypertension (HAPH). Methods: Eight male patients with HAPH aged 52-67 years
(58.57±2.41), were studied in National Center in Bishkek (760 meter above sea level). The
methods used were clinical examination, lung function tests, ECG, catheterization of pulmonary
artery, six ­ minute walk test and all patients filled in "KC questionnaire" for quality of life after
1 month sildenafil therapy. Sildenafil was given orally in a dose 50 mg thrice a day. Results: The
mean pulmonary artery pressure before therapy was 35.28±3.45 mm Hg. The six-minute walked
distance increased from 412.85±34.7 m at baseline to 483.85±24.36 m after 1 month of sildenafil
therapy. In individual analyses of oxygen saturation have a tendency to increase in all patients (up
to 2 %). The quality of life significantly improves in all 8 patients. Conclusion: We demonstrate
that 30 days sildenafil treatment in patient with HAPH significantly improves quality of life and
increased tolerance to exercise capacity.
