Note to teachers: Please ensure that all students you are entering for a Piano Graduation in Series 2, 2005 are also submitted for a Piano Graduation
Concert through this form.
· Please ensure that all details are completed in full for each student..
· Families are able to choose the time they would like to attend Piano Graduation Concerts (choice of 1:30, 3:00 or 4:30). The allocation of their time
will be confirmed at the Graduations on Sunday 21 August.
· As Piano Graduation Concerts and presentation of certificates is a special occasion, please ensure that the nominated piece to be performed is
suitably polished.
· This form should be submitted with your Piano Graduation Series One Entry Form. Closing Date Friday 15 July.
· The one payment for Graduation includes the Graduation and this Celebration.
