HOTEL REQUIREMENTS
The Westin Prince Hotel Toronto, Ontario, Canada
We have pre-arranged a block of guestrooms for this conference at the Westin Prince
Hotel at a preferred rate of C$160.00 per night, based on single or double occupan-
cy. These will be assigned on a first come first serve basis. Please indicate below
whether you would like us to reserve you a guestroom. The cut off date for booking
a guestroom at the preferred rate is March 4, 2003. After this deadline, you are
responsible for making your own overnight accommodation arrangements and the
preferred rate is not guaranteed. Please contact the hotel directly at (416) 444-2511

attendance must be communicated in writing to the Urgent Care Association no
later than March 4, 2003. Failure to cancel by March 4, 2003, will result in a
charge equivalent to one (1) night's accommodation to your credit card.

REGISTRATION FEE:
C$300.00 or US$200.00/person
The registration fee of C$300.00 or US$200.00 per person being charged for
this conference covers the cost of attendance to all sessions during the two day
conference and materials, breakfast, lunch and breaks for the same. (The
registration does not include overnight accommodation.)
This registration fee is payable by cheque only. Please mail completed
registration form (with payment only) to the following address. You will receive a
confirmation letter approximately four weeks prior to the conference date.
