This is an application for credit, or terms. Please read all parts of this agreement carefully, and have it explained to your full and complete
satisfaction prior to signing.
In connection with my application for credit with MLN Sports Group LLC ("MLN"), I hereby agree to authorize MLN to make appropriate credit checks and to allow MLN, its
agents, or assigns, to speak to the people whom have been identified above, and to research any statement(s) made herein or in attached documents within the scope of a
general credit check. Should I be approved, this application will become my contract. In consideration of the issuance of terms or credit by MLN Sports Group LLC ("MLN"), on
behalf of the company whose name appears on the first page of this document, I further understand that all accounts of the Company are due and payable IN FULL on terms
of NET 15 from the date of billing with what is, while subject to amendment in future by notification in writing, currently a prime + 7% interest charge and late fees of $20.00
for any balances unpaid past thirty (30) days. I agree to pay any and all interest, late fees, and/or collection(s) charges and reasonable fees of professionals needed to
recover my company's debts should my company fail to pay what is owed. If I dispute a charge, I will do so in writing within the fifteen(15) days of billing to avoid further
charges and/or penalties on challenged item(s). MLN will investigate all disputes and is the final determinator of the resolution of disputed charges. Should I/we dispute a
charge, I further agree to commit my company to binding arbitration to resolve any disputes with MLN, and to govern all contracts under the laws of the State of Florida. I
understand that the agent providing this, or any other form, contract, or agreement is an independent contractor providing their services as a facilitator only, and that only the
written word produced by MLN in official documents has any meaning or validity to my company's relationship with MLN. Any verbal representations by the agent are theirs,
and hold no value unless backed in writing by MLN. I further understand that my relationship with the agent is sole and separate, and that any disputes or disagreements
relative to my account that involve the agent are my responsibility to resolve with the agent. By my signature below, I bind my company to the payment of any debts to MLN
incurred, and warrant that any treatment of said debt shall be as a senior obligation of my company. MLN Sports Group LLC reserves the right to cancel my account for any
reason, at any time, with seven (7) days prior written notice. I understand that, should my company's account be cancelled for any reason, all balances are due and payable
in full at the time of cancellation.
