Training Course Registration
Registration Procedure
To register for courses, please complete the form at the end of this section, sign and date it, then fax or
mail the signed form to Jill Johnson using the Contact Information for Training Coordinator, below.
Payment
Payment in full must be received prior to the course start date. Please make your check payable to
SurgeWorks in United States dollars. Credit card payments (MasterCard or VISA only) are accepted.
Purchase orders (corporate or government) are accepted as payment; however, they must be received
prior to the course start date. The payment address is listed in our Contact Information below.
For reporting tax information, SurgeWorks federal tax ID number is 87-0538191.

Confirmation
All registrations will be acknowledged by phone or email. If you do not receive confirmation within 4 days,
please contact Ms. Johnson by phone or email immediately.
Cancellations, refunds, and substitutions
We understand that emergencies and illnesses happen. We ask for the earliest possible notice of a
cancellation. Our cancellation policy is as follows:
o More than 14 days prior to course start date 100% refund
o 7-14 days prior 50% refund
o Less than 7 days prior no refund.
You may, however, transfer your registration to another person at any time. Please send your cancellation
or transfer request to Ms. Johnson by fax or email.

SurgeWorks is not satisfied unless you are. If you're not able to apply the concepts, skills and techniques
you learned in our course, contact us right way. We'll issue you a refund or arrange for you to attend
another SurgeWorks course free of charge. That's our guarantee.
